By submitting this form, uploading content, and/or providing my initials below, I grant Zendo Project a perpetual, worldwide, royalty-free, nonexclusive license to use, reproduce, edit, adapt, publish, display, distribute, and otherwise use my testimonial, photographs, videos, and other submitted content (collectively, the "Content") for charitable, educational, fundraising, promotional, and related nonprofit purposes, in any media now known or later developed, including websites, social media, newsletters, annual reports, fundraising materials, and other print or digital communications. I represent that I own or have permission to submit the Content and grant this license. I understand that I will not receive compensation for the use of the Content. I may revoke this authorization for future uses by providing written notice to Zendo Project, but any materials already published or distributed may continue to be used.
